Medical Forms

The Boy Scouts of America requires that all youth and adult members have an annual medical evaluation. Our troop allows a 1-month grace period beyond the evaluation form's expiration date for our own local camping/activities only. High Adventure trips and Scout Council camps require a medical evaluation within the last 12 months.

These forms are available under the Forms tab. Please turn them into Yancey and Jenn Stam.
