Medical Forms

Please get your medical forms turned in to Michelle Hinze in a timely manner. These forms are available under the forms tab.Physicals are required on a yearly basis if you will be attending week long camps that involve high adventure activies (Utah) or if you are  over 40yrs old. If you will be attending resident camps(Camp Phillips,weekend camps) physicals are good for 3 years. All Physicals are due within 30 days of your due date as per Troop 116's bylaws.
